• Comprehensive Model of Care is the framework for the clinical decision- making process. It guides the therapist through the evaluation and treatment process to ensure that the comprehensive needs of the patient are met.
• Core Pathways assist the therapist with patient management in especially challenging areas that are often encountered in long term care. These include Dementia Management, Falls and Balance, Medically Complex Patient, and Short Term Rehab. All are based on the comprehensive Model of Care and enable the therapist to adequately evaluate and adapt treatment strategies for successful outcomes with special populations.
• Our Specialty Pathways assist the therapist in addressing specific issues that can be significant barriers to a good quality of life. These resources are ever growing as the needs of our patients and therapists expand. Specialty Pathways address topics such as contracture management, dementia management for SLP's, edema management, oral care, medication management, rapid recovery, pain management, urinary incontinence, and wound care.
