Jeff Clayton, PTCEOJeff was the Vice President of CHS before being named CEO in 2011. Before coming to CHS, Jeff worked as a district manager, director of operations, regional manager, and treating clinician. Jeff’s dedication to and personal interest in clinical development brought him to CHS, where he felt the environment was conducive to creating an organization that specialized in continuing education for the therapy team.
Jeff believes that in order to do what’s best for our patients we have to create strong clinicians. He is dedicated to delivering a strong clinical product according to the highest professional standards in accordance with reimbursement guidelines. His vision for CHS is to, “Develop a company that I want to work in and that everyone else will want to work in too.” Jeff and his wife Kara are the parents of four children. |  |
 | Linda Riccio, OTRVice President of OperationsLinda Riccio, a 29-year veteran of long-term care operations and clinical/programmatic development, has a BS in Occupational Therapy and has been a national educator in neurological disorders, orthopedics, and the management of special care populations. She has a diverse skill set in clinical development and implementation resulting in fiscally responsible and meaningful programs. Linda serves many roles within Creative Health Solutions, including consulting in healthcare strategy and teaching dementia management in post acute care settings nationwide. |
| Jan Bays, PTVice President of Clinical ServicesJan is a graduate of Indiana University. She has spent her career focusing on geriatric rehabilitation in the long term care and home health industry. She has served as a clinician, program director and clinical specialist prior to joining CHS. Her special interests are wound care, dementia management, adaptive seating, and patient management strategies. Her passion is client-focused, comprehensive therapy. |  |

Rhoades, PT, DPT, MSDirector of Corporate Compliance and DenialsP.J. has previously served in regional and national roles as a clinical educator specializing in cardiopulmonary, neurological and geriatric rehabilitation. She is also a former adjunct faculty member at the University of Indianapolis. Her family includes a son and daughter and she enjoys most outdoor activities. |
| Natalie Scott, MA, CCC-SLPClinical SpecialistNatalie joined CHS in 2002, serving as a treating clinician and regional manager before accepting the clinical specialist role. She was the first therapist in Indiana to receive her Vital Stim certification in 2004. She has special interest in the treatment of dementia, tracheostomies and dysphagia. She enjoys spending family time with her husband and son. |  |

She has a background in psychiatric settings but has worked in geriatric care for the last ten years, eight of which has been spent with CHS. Jenna, her husband and her two children enjoy traveling, boating, and playing board games together. |
| Daren Bootcheck, MS, OTRRegional ManagerDaren received his education from Purdue University and the University of Indianapolis. He has spent his 17-year career in hospital and acute rehab settings as well as long term care. Daren specializes in restraint reduction, positioning and manual therapy techniques. He and his family are active in their church and are huge Boilermaker fans! |  |

She has experience in acute care, home health, and worked as consultant for a construction company to ensure handicap accessibility for home construction. Terrie is married with four children and two grandchildren. |

She has regional management experience in the pharmaceutical, home health, and marketing aspects of LTC. Receiving her education from Ball State and Brown Mackie Colleges, she is excited to put her experience to work in the rehab industry. She resides in New Castle with her husband Dan and dogs Lucy and Daisy. |  |
